Requesting an Offical Transcript
In order to obtain an official transcript from our office we will need a signed request from the student containing the following information:

  • Name
  • Address
  • Day-time telephone number
  • Social Security Number
  • The number of transcripts needed
  • Where they are to be sent
  • Your signature

Our Transcript Request Form (PDF) is our preferred method of obtaining this information to ensure timely processing.

Fees/General Information
Official transcripts will be mailed upon receipt of a signed, written request from the student/alumni.

  • A $10 processing fee will be charged for each transcript request. Each request will be granted one (1) copy. Each additional copy will be assessed $5 per copy.
  • Transcripts are mailed via the United States Postal Service. A second mailing of a transcript due to mishandling by the US Postal Service or the receiving party will require a new request form and fee.
  • Walk-in or rushed requests are charged an additional $30.00 per copy. Rush options are Pickup or FedEx Overnight.
  • Official transcripts will not be released until all accounts are paid in full.

Faxing a Transcript Request
You may fax the Transcript Request Form in (with a Visa or MasterCard number, expiration date, and 3-digit security code) to (501) 318-2624.

Mailing a Transcript Request
You can mail the Transcript Request Form in with a check made payable to "Applied Life Christian College."
